I agree that a form is probably the most important element to a website design, its most likely the primary goal for the site even being built. designing a form to be engaging and not compete with the design is a challenge and here are some great examples to guide you in the right direction.

I would just add a warning don’t get so creative that a user doesn’t really understand what to do. One of my favorite forms I ever designed was this cool flash form that the fields animated out and the font was a custom font and not one fomr was filled out when I changed the form to a typical form most people are used to seeing then leads started to come in.
So be creative but don’t get carried away. its a tough balance –
